Aged Care Assessment Teams
Aged Care Assessment Teams (ACATs) help older people and their carers work out what kind of care will best meet their needs when they are no longer able to manage at home without assistance. ACATs provide information on suitable care options and can help arrange access or referral to appropriate residential or community care services such as Home and Community Care (HACC). An ACAT assessment and approval is required before people can access residential aged care, Community Aged Care Packages (CACPs) or Extended Aged Care at Home (EACH) Packages.
Community Aged Care Packages
Community Aged Care Packages (CACP) are a key element in ensuring that staying at home, rather than entering residential care, is a real option for those older Australians who choose to do so. A CACP is an individually tailored package of care services which helps people with lots of different care needs to stay living at home by co-ordinating the different services they need.
Community Transport Organisation
The CTO is a peak body representing providers of Community Transport services across New South Wales, Australia. Community Transport provides non-profit passenger transport services that cater to the needs of people who cannot make use of existing private or mainstream public transport systems. Across NSW, Community Transport operators provide services using buses, minibuses and cars driven by both paid and voluntary workers in metropolitan, regional and rural areas. Many of our services are provided in partnership with commercial bus and taxi services.
Department of Ageing, Disability & Home Care - Attendant Care Program
The Attendant Care Program aims to enhance the abilities of people with functional impairments to live as independently as they are able. By helping people to be more independent at home and in the community, the Attendant Care Program aims to prevent their premature admission to long-term and restrictive institutional environments. The Program targets people, aged 16 to 64 years, with a physical disability, who have the capacity to manage and direct their own carers, and require between 15 and 34 hours per week of personal care support. People with a physical disability caused by spinal cord injury, cerebral palsy, limb injury/amputee, stroke/ cerebrovascular accident, multiple sclerosis, muscular dystrophy, polio and spinal bifida are eligible.
Extended Aged Care at Home Package
The Extended Aged Care at Home (EACH) program assists frail aged people to remain in their homes, supported by high level care through an approved service provider. Packages are flexible and coordinated to suit the individual's particular needs, and may include services such as nursing, domestic assistance, in-home respite, personal care, transport and social support. To access an EACH Package, an older person must be assessed and approved as eligible for high level residential care by an Aged Care Assessment Team (ACAT).
Home and Community Care Program
The Home and Community Care (HACC) Program is a central element of the Australian Government's aged care policy, providing community care services to frail aged and younger people with disabilities, and their carers. The aims of the HACC Program are: to provide a comprehensive, coordinated and integrated range of basic maintenance and support services for frail aged people, people with a disability and their carers; and to support these people to be more independent at home and in the community, thereby enhancing their quality of life and/or preventing their inappropriate admission to long term residential care.
Home Care Service of NSW
The Home Care Service of NSW is one of a number of Home and Community Care providers subsidised by the Australian and NSW Governments to help people with a disability, older people and their carers to live independently in their own homes. Home Care helps people to live independently in their own homes. Services include: domestic assistance, personal care, and respite care.
IDEAS Inc: Information on Disability - Equipment Access Services
IDEAS Inc is a non-profit organisation that has developed over a twenty year involvement in the provision of information to people with a disability. The term Disability is wide ranging and as such we maintain up-to-date information on physical and intellectual disabilities, visual impairment, hearing impairment, speech/language difficulties, psychiatric, genetic, as well as issues around ageing and acquired brain injury and other conditions.
Independent Living Centre
The Centre displays a comprehensive range of products and equipment to assist with daily living activities. We are located at Parramatta, NSW, Australia. You can try out products and equipment and select those most suitable prior to purchase. We can then provide you with the details of suppliers. Please note that we do not sell items, we only display them. We are pleased to offer personal assistance and advice on products and equipment. However, it is best to make an appointment before you visit to ensure you get our full attention. Our health professionals, primarily occupational therapists, can discuss practical solutions to everyday problems with you. For people with disabilities, older people, their carers and family members, there is no charge for this personal service.
It's Your Life - Home Care Resources
As you age, the task of looking after yourself and maintaining your home becomes more onerous. If the burden becomes too great, a range of services are available to help you stay at home. Assistance is also available if you are caring for an older person or someone with a disability. Your circumstances and requirements may influence the cost and your eligibility to receive particular assistance.
NSW Meals on Wheels Association
Meals on Wheels services are operated by a combined paid and volunteer workforce, delivering meals to frail aged people, people with a disability, and their carers. The service provides crucial support for people who need assistance with meals but want to live independent lives in their own homes.
Personal Alert, Night & Day, At Home
PANDAH is a smart Personal and Home Alert System designed for people who want added security and reassurance, especially when living alone. TAD - Technical Aid to the Disabled
TAD NSW is a charity organisation that uses volunteers dedicated to the design, construction and provision of aids for people with disabilities. Members of TAD provide a resource pool comprising of a range of design, engineering, rehabilitation, computer, therapy and other professional and technical skills.
